Outpatient Services
Our Outpatient Behavioral Health Services provide comprehensive, community-based care for individuals experiencing a wide range of mental health conditions. In these settings, individuals work closely with trained clinicians to develop effective coping skills, strengthen recovery, and improve overall wellness—while continuing to meet family, work, and social responsibilities.
Services are individualized to meet each person’s unique needs and may include individual therapy, specialized group therapy, and family support
Outpatient services are available through our county clinics. Individuals may access care by contacting our Access to Care Team, who assist in connecting them with the most appropriate mental health services.
Building Stronger Communities
Case Management
Case Management Services support individuals with serious mental illness by coordinating care and advocating for essential resources. Case managers assist with transportation, housing, daily living skills, and service coordination, helping individuals access cost-effective supports that promote long-term stability, wellness, and recovery.
Assertive Community Treatment (ACT) Team
The Assertive Community Treatment (ACT) Team provides intensive, community-based mental health services through a multidisciplinary approach. ACT teams deliver coordinated, individualized care while working closely with individuals and their families to strengthen formal and informal support systems that promote stability, recovery, and independence.
Day Treatment Programs
Our Day Treatment Programs include Adult Intensive Day Treatment and Rehabilitation Day Treatment. Adult Intensive Day Treatment offers structured, time-limited, and goal-oriented services focused on reducing or stabilizing symptoms of serious mental illness. Rehabilitation Day Treatment provides long-term recovery support aimed at improving daily functioning, strengthening illness management skills, restoring self-worth, and helping individuals achieve meaningful personal, family, and community goals.
Stepping Up Case Management Services
Stepping Up Case Management Services provide coordinated, person-centered support for individuals with complex mental health needs who are involved in, or at risk of involvement in, the criminal justice system. These services focus on stabilization, reduced justice system involvement, and improved quality of life through comprehensive assessments, individualized service planning, care coordination, and collaboration across behavioral health, medical, housing, and justice systems. By addressing underlying mental health needs rather than relying on incarceration, Stepping Up promotes recovery, community stability, and safer outcomes for individuals and communities.
